Geriatric Assessment Program (GAP)

The Geriatric Assessment Program (GAP) at Euclid Hospital is a consultative resource for elderly patients, family members and caregivers, and primary physicians (PCPs).  It provides comprehensive geriatric assessment of an elderly patient's health issues, in the context of their social and family needs.  GAP also helps with access to appropriate community and private supports, and provides education and information about common geriatric health problems and concerns.

When is an assessment needed?

Older individuals in need of a comprehensive assessment could be experiencing any combination of the following symptoms:

  • Multiple health problems
  • Confusion or memory loss
  • Behavioral change, including depression or anxiety
  • Difficulty in performing daily activities 
  • Weakness, caused by deconditioning or other health problems
  • Nutritional concerns, including unexplained weight loss
  • Problems resulting from multiple medications
  • Uncertainty about one's ability to live independently 

What can you expect from an assessment?

  • A clearer understanding of your problems and issues
  • A comprehensive assessment of your care needs
  • A list of options to consider regarding your health issues
  • A comprehensive plan for your continuity of care
  • Help with difficult decisions regarding living environment and home safety
  • Referrals to all appropriate services
  • Consultation to assist your PCP

There are 3 main focus areas:

  • Health
  • Functional Status and Self-Care Ability
  • Safety

The needs of both patients and families/caregivers are always considered.

  • Pharmacist

What is the cost?

Fees are covered by Medicaid/Medicare supplemental insurance that Euclid Hospital bills directly.  You are only responsible for a deductible and/or co-payment.  Physicians and other specialists may bill separately through referral, but coverage of additional services will be reviewed with patients and families/ caregivers before being obtained.
